Clinical Rest & Restorative Barrier Care
The Clinical Reset: Cortisol, Circadian Rhythms & Reclaiming Rest
To get to the root of why so many of us experience nighttime awakenings and daytime burnout, we sourced Dr. Avinesh Singh Bhar, MD, MBA,a fellowship-trained, board-certified sleep, critical care, and pulmonary specialist who founded the virtual sleep practice SLIIIP to deliver accessible, guideline-based care.
Q: The 3 AM Wake-Up & Cortisol: From a medical perspective, how does daytime stress impact nighttime rest, and what clinically causes that "wired but tired" phenomenon?
Dr. Bhar: Daytime stress, especially related to thought streams, directly impacts our ability to fall and stay asleep. The need for quality sleep is essential when stressed. Most of us briefly awaken during the night, turn over, and drift back to sleep. However, if that brief awakening triggers a stressful thought (βI have to pay that billβ or βmy project submission is due at 9 AMβ), it reduces the ability to fall back to sleep. While we blame stress on poor sleep, your body is geared toward getting adequate rest. The question should beβwhy am I waking up repeatedly? In my opinion, the βwired but tiredβ phenomenon is related to the underlying cause of the awakening: a surge of cortisol or sympathetic nervous system activation due to sleep-disordered breathing, i.e., sleep apnea. Many of us have sleep apnea and either donβt know it yet or donβt want to know it.
Q: Nervous System Down-Regulation: What are 1 or 2 actionable steps individuals can take to effectively down-regulate their nervous system before bed (or during a middle-of-the-night awakening)?
Dr. Bhar: Wind down 30β60 minutes before bedtime with simple steps such as low lighting, reading, or relaxation exercises like deep breathing to blunt the stress response.
"Sleep is a natural physiological process like urination, hunger, or thirst. If I told you that you needed an elaborate routine of supplements, eye masks, and temperature-controlled bedding just to sit down for a meal or use the restroom, youβd simply call me crazy. So why do we accept that sleep is any different?" β Dr. Avinesh Singh Bhar
It is simply because we fill our heads with stories to make sense of things before we understand our bodies. We have that knowledge nowβdon't struggle in the darkness alone.
Q: Expanding Access through Virtual Care: How is telehealth changing the game for treating stress-induced sleep disruptions, and what is the biggest advantage for a patient transitioning to virtual care through SLIIIP?
Dr. Bhar: Most sleep care is about listening and solving problems. With home sleep testing and cloud-based monitoring of therapy, virtual care is the best way to address sleep issues. The U.S. produces fewer than 200 sleep doctors a year, leading to long wait times nationwide. Virtual care democratizes access regardless of location. We are in-network with most insurance plans and have competitive cash rates as well.
Q: About SLIIIP: How does SLIIIP help patients break these cycles, and what is your current availability?
Dr. Bhar: As a virtual sleep practice led by board-certified sleep physicians, SLIIIP provides comprehensive sleep careβfrom insomnia and sleep apnea to nightmares and sleepwalkingβfor patients from ages 13 to 99 across all 50 states with same-week availability.
